Risk Description Examples
5 - Critical Critical defects rendering asset physically unsound / functionally unfit for purpose with high risk of failure. Immediate action required. • Exterior building component at imminent risk of falling:  e.g.   Loose piece of flashing, dislodged bricks, etc.
• Collapsed or Termite damaged flooring that a person could fall through.
• Damaged / leaning retaining will at imminent risk of collapse
• Water ingress into electrical equipment
• Corroded or loose exterior guard rails, ladders, grid mat at imminent risk of failure
4 - High Major defects badly deteriorated with potential structural impact. Essential maintenance / repair work required. • Cracked brickwork which could become unstable if not repaired
• Water leakage into ceilings / walls (not impacting on electrical safety)
• Corroded steel work / fasteners that could fail if left unattended
• Signs of active termite damage (not at imminent risk of collapse)
3 - Medium Significant defects
Still stable / secure. At risk from further damage / deterioration. Maintenance / repair work may be required.  Programmed monitoring is required.
• Concrete spalling (not at risk of collapse or impact on personnel)
• Cracked brickwork >5mm but still stable
• Corroded steel work / fasteners minor flaking yet still secure
2 - Low Minor defects or deterioration. Still stable / secure. Desirable maintenance / repair work / programmed monitoring is required. Defects noted for attention • Minor leakage through window seals
• Water stains with no current wetness
• Signs of loss of corrosion protection on structural steel (paint / galvanising)
• Minor cracks in brickwork / concrete structural elements (<5mm)
• Cracked paving
1 - Very Low No defects. As-new condition and appearance. Optional maintenance / repair work. N/A

Risk Description
5 - Critical Risk
Significant potential for adverse impact on the life safety of occupants, Essential maintenance / repair work required. e.g. Failed Pump, Multiple EEL lighting failure (No lit path to Exit), multiple detectors failed, System Impairment.
4 - High Risk
Functional but operation is impacted, Essential maintenance / repair work required. Potential minor impact only e.g. Fire Panel alarm, Vesda Failure (residual detection still present), Multiple EEL lighting failures (some residual lighting), Fire door does not function, Fire Panel Battery Failure.
3 - Medium Risk
Functional, Repair may be required. Programmed monitoring required. Potential Minor Impact with Low Exposure e.g. Asset beyond recommended service life but still working , Leaking Fire Hose Reel, VESDA filter replacement, VESDA life cycle replacement (10 yrs.), Fire Panel battery LCR (still functioning), Block Zone Diagram missing.
2 - Low Risk
Functional, Desirable maintenance / repair work. Possible Minor Impact with Unlikely Exposure e.g. Missing Fire Extinguisher (others in vicinity), Individual EEL failure (path to exit still clear and illuminated).
1 - Very Low Risk
Functional, Optional maintenance / repair work. Negligible Impact with Rare Exposure e.g. Faded Fire Extinguisher sign (still visible).

• Cooling operating at reduced capacity due to fault but there is available redundancy

• Low risk to health & safety (High risk call GOC for Reactive Maintenance)

3 - Medium Significant defects but still functional, stable / secure. At risk from further damage / deterioration. Necessary maintenance / repair work / programmed monitoring is required.
• Asset beyond recommended service life but still working
• Very low risk to health & safety (High risk call GOC for Reactive Maintenance)
